Please use this form for your event safety + risk planning. This form needs to be completed in one sitting as it doesn't save in draft form. It is intended to be a plan, so you don't have to have already completed the actions that arise from this planning before you submit the form.

During your event, please report incidents + near misses. This is valuable information that helps us all grow in our intelligence about what makes a great event.

CONSIDERING THE NEEDS OF VULNERABLE PEOPLE

RESPONSIBLE PEOPLE + SUPERVISION

*NOTE: Responsible adult means an Oceans Team Member who has completed the relevant OnTeam induction and training/s. Minimum ratio's include Toddlers: 1:5 | Kindy 1:7 | Primary 1:8 | Youth 1:8. These should be increased as appropriate for high risk activities. A team of 2-3 responsible people (point person) is recommended for Adult events.

This is your overall point person, ultimately responsible for the event. They must be in attendance and available. Best practice is to have a second person, who assists and supports; and steps in where the responsible person becomes unavailable for any reason. 

E.g. First Aid officer is Tim, who is a nurse. First aid officer is Jack who has a current first aid certificate. First Aid officer is John who hasn't got a formal qualification, but has previous training + experience.
